The All charges and usage is found under Billing- Reports- Detail. From there, you'll scroll down to "All wireless charges and usage." To the right of this, you'll see a pen, a calendar, and a trash can. 

If you click on the pen, that will let you modify your fields you see. It defaults to a lot more than you have on your reports sheet, so you can click the arrow to remove them from the report. You can also add more fields as well (UDL's). 
After you edit your fields, you can then click on the calendar to choose what dates you want to run the report from.
